Wilson’s disease is a genetic disorder that is relatively uncommon and mostly affects the liver and brain. There are two main types of symptoms, which are generally called the neurological and the hepatic types. The neurological type has progressive neurological symptoms that worsen over time, while the hepatic type usually results in a triad of symptoms known as Kayser-Fleischer rings, hepatosplenomegaly and jaundice. Mortality is higher in the hepatic type, but it is unclear if patients with this type can ever recover. One of the most definitive tests for Wilson’s disease is the measurement of the ceruloplasmin level in the blood. Given that the diagnosis is usually delayed in patients with hepatic symptoms, a liver biopsy is required to determine the stage of the disease, which carries a much higher mortality risk for patients with hepatic symptoms. However, given that liver transplantation has traditionally been the only treatment option for patients with Wilson’s disease with end-stage liver disease, most centers do not currently provide this treatment to patients with hepatic symptoms. In this article, we review the current state of the disease and the treatment options available for patients with hepatic symptoms.
