Sodium-calcium exchanger (NCX) dysfunction has been shown to play a major role in the pathophysiology of heart failure. However, very few studies have assessed the effect of pharmacological compounds on NCX activity. The present study was designed to investigate the molecular and functional alteration of NCX activity in an experimental model of severe heart failure. Myocardial infarction was induced by coronary ligation in pigs. In nine infarcted animals, we demonstrated that the number of NCX currents was dramatically decreased. Importantly, six normal animals were found to have a lower NCX activity. The positive inotropic and lusitropic effects of the novel NCX inhibitor EKA-7943 were inhibited by NCX activation via isoproterenol. We then evaluated the effect of two drugs targeting NCX activity, EKA-7943 and ranolazine, in pig models of infarction. Infarct size in both groups decreased to comparable values. After the isoproterenol test, left ventricular stroke volume was still decreased in the ranolazine group but increased in the EKA-7943 group. Phosphorylated myosin light chain 1 was reduced in the isoproterenol group only and ranolazine increased it. It is therefore concluded that EKA-7943 regulates NCX activity and hence raises the stroke volume. This beneficial effect on cardiac contractility may partly explain its good tolerance and safety.Part 2: How the 2008 Olympic marathon could be won—and lost
